    Templates to be used in homes offered section

    Please copy and paste the following, and e-mail us the completed form to [email protected] in the first instance, or message (PM) a member of staff:


    Template to be used if you are interested in adopting a dog/puppy

    What breed(s) of dog are you interested in adopting?
    How many (e.g. one, pair):
    Sex:
    Colour/pattern:
    Approximate age*:

    *Please note rescues do not allow puppies to be adopted younger than 8 weeks of age. Animal Lifeline UK recommends that the puppy is at least 10 weeks of age at the time of adoption and should be fully vaccinated and health checked.

    Do you live in a house or flat?

    If rented accommodation, does your landlord allow pets? (The rescue may require proof of this)

    How many rooms are in your house?

    Do you have a garden?

    Is it securely fenced?

    If so how high at the lowest point (this is very often the gate)?

    Is there a main road near your house?

    How long will the dog/puppy be left alone for?

    Would you consider adopting a dog/puppy with medical problems?

    Would you consider a dog/puppy with behavioural problems? (Please note that even if a dog/puppy doesn't show any behavioural problems in rescue they can develop them in the future).

    Would you consider a dog/puppy with toileting problems? (Even though a dog/puppy may show no sign of toileting issues in rescue any dog/puppy can develop toileting problems).

    Are you prepared to take your dog/puppy to training classes if required/recommended?

    How much exercise will your new dog/puppy get? (E.g. Half hour walk in the morning, one hour walk in the evening).

    Are you prepared to pay an adoption donation/fee? (The amount will depend on the individual rescue).

    Name of the vets you intend to use:

    Are you happy for a home check to be carried out?

    How many adults reside in your home? Please include ages:

    How many children reside in your home? Please include ages:

    Do any of the above have any allergies?

    Do any children visit your home? If so how many and frequency? Please include ages:
